When you submit a consultation request, we collect your first name, last name, email address, phone number, and your preferred consultation location. When you reply to our scheduling text messages, we collect the content of those messages and the timestamps. By submitting our consultation form and checking the consent box, you give CWLI permission to contact you by text message and email at the number and address you provide. Message and data rates may apply. You may opt out of text messages at any time by replying STOP. Opting out of text messages does not affect your ability to call our office or receive other communications you have specifically requested.
Protected Health Information (HIPAA)
Information you share with us as a patient — including health history, lab results, and clinical notes — is protected health information governed by the Health Insurance Portability and Accountability Act (HIPAA) and our separate Notice of Privacy Practices, which is provided at your first appointment. The website privacy practices described here apply to information collected before you become a patient.
